Data Entry (transcription) Errors At The Pharmacy

Hi, I recently read a few articles about pharmacy technicians making data entry(transcription) errors when they receive the prescription from the patient.  I was wondering do you know of any studies conducted that mapped out the most common transcription errors made at the pharmacy, for example: accidentally leaving the quantity dispensed entry null, or typing in extra 0's.  Your help would be greatly appreciated.   Thanks.

*I have been thinking a out your questions, and maybe I misunderstood your purpose.

All states license pharmacists. Perhaps you should ask for information from the licensing bureau in your state. If they license or regulate in some way pharmacy aides or pharmacy technicians, they will be able to provide you with the requirements and responsibilities for these peaople.

Also, keep in mind that the licensed pharmacist is respsombile for all medicines leaving his shop. It doesn't matter who actually types the label, or who misreads the prescription. Only a licensed pharmacist is allowed to dispense medicines, or to fill a prescription. So, if he designates an employee to do this for him, he is still responsible. Remember that the "buck stops here", and Mr. Truman meant that only one person is responsible.

The state licensing agency may have some public records that can help you. These might be records of license suspensions or other actions because of this kind of error. It is worth checking out.
